Responsibilities

The Community Health Worker will build trusting patient relationships, conduct in-home and virtual visits using the 5M's framework, advocate for patients, connect them with community resources, navigate and coordinate care, and provide wellness and life skills coaching. Responsibilities include screening for Social Determinants of Health needs, maintaining a resource list, performing patient outreach and education, coordinating clinical orders, collaborating with the interdisciplinary care team, and educating patients' families and caregivers.
