Candidates must have 3+ years of active, current licensure as an LCSW, LPC, LMFT, or other relevant healthcare credential in good standing. Proven experience in care management, patient advocacy, or healthcare navigation is required, along with a deep understanding of Social Determinants of Health and experience working with diverse patient populations. Strong clinical skills, exceptional organizational abilities, and technical proficiency are essential.
The Healthcare Advocate will learn the Solace systems, tools, technology, and expectations while providing unique expertise in every interaction. They will build strong, trusting relationships with Medicare patients, identifying and prioritizing their needs to maintain a streamlined care continuum. Developing comprehensive patient care plans that address social determinants of health and working collaboratively with Medicare patients to build future systems are also key responsibilities.
