Requirements

  • Active, unrestricted license(s) in coverage area(s) (New Hampshire)
  • Board certification (required)
  • Multi-state licenses (preferred)
  • Able to cover the following primary locations: New Hampshire
  • NP or PA credential (implied by role) and corresponding licensure

Responsibilities

  • Conduct in-home health evaluations (~45-minute visits) including medication and medical history review and physical examination
  • Perform additional diagnostic tests ordered by the member’s health plan (e.g., diabetic eye exams, spirometry) as part of the evaluation
  • Use an iPad and the intuitive clinical workflow; conduct virtual visits as needed
  • Provide all Signify Health services as indicated, including Diagnostic & Preventive Services (e.g., spirometry, DEE, bone density screening)
  • Travel locally and within licensed states as needed; work primarily in members’ private residences
  • Build trusted relationships with members, educating and guiding them to appropriate care without prescribing medications, ordering labs, or altering current treatment regimens unless required by the plan
