Requirements

Candidates must have a track record of success in health-care-related engagement, sales, and/or marketing. Strong verbal and written communication skills, along with proficiency in Excel and EHR/EMR platforms, are required. A dedication to serving the community, the ability to operate independently in a fast-paced environment, and an understanding of Medicare and Medicare Advantage programs are essential. Access to reliable transportation and US work authorization are mandatory. Fluency in Spanish is preferred.

Responsibilities

The Patient Engagement Specialist will conduct telephonic and in-person outreach to engage patients in the care model, including home visits, and assist them in accessing care or finding appropriate primary care settings. Responsibilities include spending half of the time in the community connecting with patients, responding to escalations, documenting interactions in electronic medical records while maintaining HIPAA standards, and communicating complex topics between patients, caregivers, providers, and community organizations. The specialist will also partner with patients and their support persons to identify goals and address barriers, foster relationships with patients and caregivers, collaborate with other healthcare professionals to support complex patient needs, and educate patients about wrap-around services.
