Position Overview

NeuroFlow is seeking a dedicated individual to join our Care Navigation team. This role involves directly assisting patients and their providers in connecting to mental health care. You will respond to referrals and proactively reach out to patients identified with mental health needs, aiming to minimize the challenges patients face in accessing care and ensuring a seamless process for connecting them with their physicians.

Employment Type: Part-Time Hours: Between 9 am - 6 pm EST

NeuroFlow was founded by CEO Christopher Molaro, a West Point graduate with five years of military service, including a tour in Iraq. Returning home, he recognized significant gaps in the behavioral health system, particularly the barriers veterans and civilians encounter when seeking appropriate and timely care.

While pursuing his MBA at Wharton, Chris met co-founder Adam Pardes. They realized that even the most engaging digital mental health apps alone couldn't solve the problem; a systematic integration of behavioral health into the entire healthcare ecosystem was necessary for meaningful change. This realization led to the creation of NeuroFlow.

About the Team & Opportunity

The Care Navigation team is responsible for connecting patients with mental health care by working directly with patients and their providers. This involves responding to referrals from referring providers and proactively contacting patients identified as having mental health needs. The team's objective is to reduce barriers to care and make the process as smooth as possible for patients.

Responsibilities

Patient and Provider Interactions:

  • Engage directly with patients via phone and secure messaging to facilitate access to mental healthcare.
  • Utilize the Quartet system to identify suitable provider matches based on location, specialty, insurance, and patient preferences.
  • Conduct informed discussions with patients to understand their care needs and support their care journey.
  • Troubleshoot and resolve any issues related to patient referrals.
  • Assist providers via inbound calls to connect their patients to mental health care and resolve issues with referral submissions.
  • Receive inbound calls from case managers to create referrals and assist their patients in accessing mental health care.

Administrative Duties:

  • Document all patient and provider interactions accurately.
  • Effectively utilize Quartet technology, tools, messaging, and workflows.
  • Provide support for administrative tasks as required by the Care Navigation team.
  • Develop a comprehensive understanding of Quartet's product to successfully route patients to appropriate care.
  • Apply foundational knowledge of workflows and market nuances to assist patients in their care journey and support providers with referral submissions.

Measures of Success

  • Meet team expectations for patient engagement and referral completion.
