Requirements

Candidates should possess prior experience in case management or a healthcare setting, demonstrating proficiency in motivational interviewing techniques and the ability to multitask effectively in a dynamic environment. Strong telephone skills, including the capacity to navigate conversations and anticipate needs, along with computer technology proficiency and the ability to quickly learn new platforms, are also required. Experience with health IT systems and reports is desirable, and knowledge of community healthcare and social welfare resources is preferred. Furthermore, candidates must demonstrate a positive and respectful attitude, acknowledge patient rights, maintain patient confidentiality, and adhere to HIPAA guidelines.

Responsibilities

The Connected Care Assistant will complete scheduled shifts for member calls, chats, and internal referrals into CCP, participate in targeted outreach shifts to engage at-risk members into CCP, utilize autodialer tooling for targeted outreach, manage and triage care plan task queues, coordinate care for members and their families/caregivers as identified by the clinical team, recommend benefits available to members, initiate related cases such as Provider Match and Concierge Referrals, compose letters and mailings, obtain medical releases and records, triage callback tickets, monitor email referrals and communications, monitor and manage portals, attend huddles when requested, attend ongoing training, comply with organizational guidelines and healthcare laws, and perform other duties as assigned.
