Candidates must possess a Master's or Doctoral degree in Nursing, with a specialization in primary care, internal medicine, or geriatric medicine preferred. An active and clear RN and APRN license in AZ, CO, or TX is required at hire and must be maintained, along with a willingness to obtain multiple state licenses. Three or more years of experience as an APRN in primary care, house calls, internal medicine, family medicine, geriatrics, and/or palliative care is necessary. Proficiency in telehealth technology and EHR systems, a team player mentality, a continuous improvement mindset, a can-do attitude, and an active BLS certification at time of hire are also required.
The Nurse Practitioner will deliver exceptional care to a cohort of complex patients via telehealth, guiding and overseeing their medical management using the 5M framework. They will work closely with the patient's care team, including PCPs, specialists, and other Devoted teams, as well as collaborate with family members and caregivers to coordinate care and develop a collaborative care plan. Responsibilities include performing comprehensive assessment visits, providing post-acute transition support, conducting comprehensive diagnosis/disease reviews, medication reviews, and assessing for quality of care interventions and social/home health needs. In limited instances, the NP may provide telehealth visits to members outside their panel to support other Devoted Medical service lines, and may participate in an infrequent weekend on-call component. The role also requires maintaining accurate patient medical records and ensuring compliance with legal, ethical, and telehealth regulations.
