Requirements

Must be located and work in Pacific Standard Time. Experience in care coordination, particularly for a predominantly elderly Medicare population, is required. Strong communication skills, comfort with video and phone interactions, and proficiency with technology including electronic health records are essential. Must be an entrepreneurial self-starter committed to delivering high-quality care and building longitudinal relationships.

Responsibilities

Serve as a liaison between healthcare services and members, providing high-touch telehealth visits and supporting the clinical team. Identify care gaps and coordinate member care across the healthcare system, including managing pharmacy services, DME, home health, imaging, and laboratory testing. Prepare members for telehealth visits, manage orders, coordinate services with home health partners, and track lab and imaging results. Notify members of lab results and coordinate follow-up care as directed by clinicians. Assist with medical record collection, information gathering, and appointment coordination. Triage member issues to appropriate teams and follow up with members post ER and Dispatch Health visits. Act as an advocate for members, answer calls, return calls, review referrals, and send educational materials.
