Requirements

  • Active and unrestricted RN licensure in the state of residence (compact license)
  • 3+ years of clinical practice experience
  • Ability to work 8:00a - 5:00p EST
  • Minimum 2+ years of Care Management, discharge planning, and/or home health care coordination experience (preferred)
  • Ability to work independently
  • Excellent analytical and problem-solving skills
  • Effective communication, organizational, and interpersonal skills
  • Ability to work with computer systems and keyboarding
  • Proficiency with standard corporate software applications (MS Word, Excel, Outlook, PowerPoint, etc.)
  • ADN or BSN degree

Responsibilities

  • Assess, plan, implement, and coordinate case management activities with members to evaluate their medical needs
  • Facilitate members' overall wellness
  • Work as frontline advocates for members who cannot advocate for themselves
  • Evaluate the medical needs of members
  • Coordinate with teams virtually to collaborate and connect with members
  • Use tools to work independently and effectively communicate with teams
  • Demonstrate proficiency with proprietary applications in addition to standard corporate software applications
