Requirements

Active and unrestricted RN licensure in the state of residence (compact license)
3+ years of clinical practice experience
Ability to work 8:00a - 5:00p EST
Minimum 2+ years of Care Management, discharge planning, and/or home health care coordination experience (preferred)
Ability to work independently
Excellent analytical and problem-solving skills
Effective communication, organizational, and interpersonal skills
Ability to work with computer systems and keyboarding
Proficiency with standard corporate software applications (MS Word, Excel, Outlook, PowerPoint, etc.)
ADN or BSN degree

Responsibilities

Assess, plan, implement, and coordinate case management activities with members to evaluate their medical needs
Facilitate members' overall wellness
Work as frontline advocates for members who cannot advocate for themselves
Evaluate the medical needs of members
Coordinate with teams virtually to collaborate and connect with members
Use tools to work independently and effectively communicate with teams
Demonstrate proficiency with proprietary applications in addition to standard corporate software applications
