Requirements

Candidates must reside within a 30-mile radius of Waynesboro, Virginia, and possess a Bachelor's degree in a health or human services field or an active LPN license in Virginia. They should have two years of prior experience in healthcare and/or case management, and one year of experience working directly with individuals meeting Cardinal Care Priority Population criteria. Intermediate to advanced computer skills, including Microsoft Word, Excel, and Outlook, are necessary, along with the ability to use various electronic information applications. Exceptional communication and interpersonal skills are required, as is the ability to manage multiple priorities and travel up to 50% for field interactions.

Responsibilities

The Care Coach will evaluate member needs to optimize wellness, manage a caseload, and develop individual care plans involving the care team. They will provide specialized support for members, focusing on health-related social needs, psychosocial support, and individual health requirements. Responsibilities include ensuring members receive services in the least restrictive setting, collaborating with community resources, monitoring patient progress, and acting as the primary point of contact for the ICT to coordinate care and resolve barriers.
