About the Team

The Care Delivery team is seeking a mission-driven health coach to provide exceptional care to Maven members. This team, comprised of care coaches and care advocates, assists members in navigating digital care and accessing resources for optimal experiences and outcomes. The team emphasizes continuous learning and data-driven strategies to improve members' lives through a shared coaching model. As a Care Coach, you will coordinate care beyond the digital realm, driving behavior change and improved outcomes for higher-risk members through goal setting and program engagement.

The Care Delivery team operates remotely and provides 24/7/365 support to Maven members. The current hiring need is for shifts: Tuesday - Saturday, 3 PM - 11:30 PM EST.

Responsibilities

As a Care Coach, you will be responsible for:

  • Communicating the benefits of the Maven platform and engaging in care coaching with Maven members to maximize program opt-in rates.
  • Utilizing coaching frameworks and principles to collaboratively develop SMART goals with members, ensuring adherence and refinement within Maven’s clinical program design.
  • Conducting introductory and follow-up appointments to build longitudinal relationships through outreach and caseload management.
  • Adhering to clinical program guidelines and protocols, employing motivational interviewing skills and behavior modification techniques to foster behavior change and promote optimal outcomes.
  • Assessing member knowledge and barriers efficiently, delivering personalized health education tailored to diverse ages, backgrounds, cultures, and literacy levels.
  • Collaborating with members' external care providers, integrating their recommendations into member care plans.
  • Remotely monitoring member health data and activities, utilizing data to inform actions in accordance with established protocols and procedures.
  • Ensuring seamless access to care by coordinating care within the Maven digital network.
