The Care Delivery team is seeking a mission-driven health coach to provide exceptional care to Maven members. This team, comprised of care coaches and care advocates, assists members in navigating digital care and accessing resources for optimal experiences and outcomes. The team emphasizes continuous learning and data-driven strategies to improve members' lives through a shared coaching model. As a Care Coach, you will coordinate care beyond the digital realm, driving behavior change and improved outcomes for higher-risk members through goal setting and program engagement.
The Care Delivery team operates remotely and provides 24/7/365 support to Maven members. The current hiring need is for shifts: Tuesday - Saturday, 3 PM - 11:30 PM EST.
