Requirements

Candidates must possess an RN license in Kentucky, with 3-5 years of clinical practice experience in settings such as hospitals, ambulatory care, or outpatient clinics. A minimum of 2 years of experience with personal computers, including navigating multiple systems and MS Office Suite applications, is required. An Associate's or diploma in nursing is the minimum educational requirement, with a BSN preferred. Experience in case management, managed care, discharge planning, and caring for the Medicaid population is preferred.

Responsibilities

The Care Manager RN is responsible for driving and supporting care management and care coordination across the continuum of care, including assessment, planning, implementation, coordination, monitoring, and evaluation. This role utilizes advanced clinical judgment and critical thinking to facilitate appropriate physical and behavioral healthcare through assessment, care planning, provider coordination, and psychosocial support to ensure optimal, cost-effective outcomes and resource utilization.
