Requirements

  • Driver's License - Upon Hire
  • Preferred: High School or GED Equivalent
  • Preferred: Certified Nurse Aide (CNA) - Licensing Board
  • Preferred: An active license in the state of practice
  • Preferred: Less than 1 year in home health care or related health care field
  • Visual acuity adequate to perform position duties
  • Ability to communicate effectively with others, hear, understand, and distinguish speech and other sounds

  • Maintain confidentiality
  • Work effectively in a team environment
  • Comply with safety principles, laws, regulations, and standards (e.g., CMS, The Joint Commission, DHHS, OSHA)
  • Minimum of 4 shifts per month (PRN/as needed)

Responsibilities

  • Provides assistance to patients with their activities of daily living
  • Provides personal care for patients as directed and delegated by the nurse, according to the established plan of care
  • Provides services such as housekeeping, laundry, meal preparation, and shopping
  • Assists patient with social activities as prescribed by the plan of care
  • Maintains timely and accurate documentation
  • Reports changes in patient status to supervisor
  • Demonstrates the knowledge and skills necessary to provide care appropriate to the age of the patient served
  • Uses equipment and supplies effectively and efficiently
  • Adheres to environmental, safety, and infection policies and procedures
  • Promote Avera’s values of compassion, hospitality, and stewardship
