Requirements
- Board-certified physician with at least 3 years post-residency experience in Family Medicine, Internal Medicine, Emergency Medicine/Urgent Care
- Telehealth experience required
- Multi-state licensure preferred or willingness to obtain additional licensure
- Adept at learning and utilizing electronic health data and medical record systems, including video, telephonic, and AI platforms
- Eagerness to advocate for care equity and accessibility for vulnerable patient populations
Responsibilities
- Deliver direct care (90% focus) in virtual urgent care and observation unit via remote telehealth, including video visits, to manage urgent care complaints and avoid unnecessary hospitalizations while escalating higher acuity cases
- Provide clinical supervision, teaching, and guidance (10% focus) to a team of advanced practice providers, including joining clinical rounds and supporting challenging scenarios
- Foster a collaborative and supportive clinical environment and promote a team-based culture of caring for patients like family
- Manage patients in the virtual observation unit, including acute-on-chronic disease exacerbations, evidence-based practice, incorporating patient/family preferences, in-depth medication review/deprescribing, and ensuring optimal outcomes
- Connect patients with primary care and bridge to comprehensive care programs such as complex care, behavioral health, chronic disease management, and palliative care
