The Patient Engagement Associate II will conduct thorough research on attributed patients to understand their healthcare needs and identify potential barriers to enrollment, perform telephonic outreach to caregivers to enroll children in the program and ensure a seamless onboarding experience, and engage with other healthcare professionals involved in the care of attributed patients to facilitate enrollment and coordination of care. This role also involves occasional travel to provider offices and requires creative problem-solving skills to address patient and caregiver needs effectively.
