We partner with families, providers, and community resources to break down barriers and improve the health and lives of these children. This role focuses on enrolling and onboarding patients into the program.

Requirements

• Strong critical thinking and creative problem-solving skills
• Aptitude for collaboration and teamwork
• Research skills
• Telephonic outreach experience (preferred)
• Ability to understand and address healthcare needs
• Knowledge of pediatric healthcare (preferred)

Responsibilities

• Conduct thorough research on attributed patients to understand their healthcare needs and identify potential challenges.
• Perform telephonic outreach to caregivers of attributed patients to enroll them in the program and ensure a seamless onboarding experience.
• Perform telephonic outreach to other healthcare professionals engaging with attributed patients and their caregivers.
• Collaborate with engagement pod members to achieve enrollment and onboarding goals.
