Requirements

Candidates must possess a high school diploma or equivalent and have prior customer service or healthcare navigation experience. Experience working with diverse or underserved populations is a plus. Required skills include excellent customer service and communication, an empathetic and proactive approach, strong critical thinking and organizational abilities, proficiency with technology, and bilingual fluency in English and Spanish. Prior experience with health insurance or care coordination is also required.

Responsibilities

The Care Advocate will make over 60 outbound calls daily to engage members, motivate them to complete health actions, and re-establish contact. They will use motivational interviewing techniques to encourage behavior change, identify and resolve barriers to care, and answer questions about the Wellth Rewards program. Responsibilities also include assisting with healthcare navigation, re-engaging members who have fallen behind, communicating across multiple channels, escalating complex issues, and sharing feedback for continuous improvement.
