Requirements

  • Expertise in social work theoretical perspectives, professional standards, and FMCNA guidelines
  • Ability to create strategic plans to improve outcomes, key performance indicators, Quality Incentive Program (QIP), and Value Based Care Program
  • Skills in leadership, coaching, and development of direct reports
  • Knowledge of Human Resources processes for employee matters
  • Ability to manage staffing, orientation, and training of master social workers
  • Competence in monitoring, auditing, and ensuring compliance of documentation (e.g., Progress Notes, Comprehensive Interdisciplinary Assessments, Plans of Care)
  • Ensures master social workers maintain current licensure, continuing education per Association Social Work Board and state regulations
  • Ability to attend and participate in regional, area, facility, and team meetings
  • Maintains and improves knowledge and skills for competent and innovative practice
  • Ability to perform in the described physical demands and working conditions

Responsibilities

  • Serves as subject matter expert and collaborates with management to create strategic plans improving QIP, Value Based Care, and quality outcomes in assigned area
  • Manages execution of Social Work initiatives, interventions, standardized counseling, and education materials with facility MSWs
  • Provides support to facility staff to promote patient/provider relationships, addressing disruptive behavior and discharge processes while reviewing CMS and FMS requirements
  • Collaborates with Area Operational leadership to develop strategic plans supporting patient experience, quality improvement, initiatives, and trainings
  • Provides direction, guidance, and feedback to facility MSWs on quality, patient satisfaction, teamwork, unit culture, and employee satisfaction
  • Provides leadership, coaching, and development plans for direct reports; partners with HR on employee matters
  • Manages staffing of Social Work Services, including coverage for leaves, vacations, census increases, and vacancies
  • Manages orientation and training of new MSWs; provides ongoing communication and training for existing staff
  • Attends and participates in regional, area, facility, and team meetings; collaborates for effective relationships
  • Monitors and audits Social Work documentation to meet CMS and FMS policies
  • Ensures licensure and continuing education compliance for MSWs in assigned area; monitors direct reports' status
  • Performs other related duties as assigned
