Requirements

Candidates must be a Nurse Practitioner or Physician Assistant with current licensure in Washington state. Experience in home health or a related field is preferred, and a strong understanding of medical terminology and patient assessment skills are required.

Responsibilities

The Per Diem Clinician will conduct in-home health evaluations, lasting approximately 45 minutes, which include connecting with patients one-on-one, reviewing medication and medical history, performing physical evaluations, and administering diagnostic tests as ordered by the patient’s health plan. They will also provide education to patients and their families, conduct virtual visits when needed, and collaborate with Signify Health as a clinician colleague to fulfill the requirements of the role, while adhering to all applicable regulations and guidelines.
