Requirements

Candidates must be a Nurse Practitioner (APRN) and able to work within Pacific Standard Time (PST), Mountain Standard Time (MST), or Central Standard Time (CST) hours. Experience in virtual clinical care for a wide range of diseases and across a wide spectrum of acuity is required. The ability to diagnose, manage, and monitor decompensating patients with common readmission conditions such as heart failure, COPD, diabetes, hypertension, pneumonia, cellulitis, and urinary tract infections is necessary. Familiarity with electronic health information systems is also required.

Responsibilities

The Transitions of Care Nurse Practitioner will provide virtual clinical care for patients within the 30-day window after discharge from an acute setting. This includes conducting comprehensive post-discharge medical assessments, developing individualized care plans, and providing disease-specific clinical education. The NP will diagnose and manage acute symptoms, participate in interdisciplinary rounds to discuss high-risk patients, and collaborate with RNs, LPNs, and clinical coordinators. Responsibilities also include assisting with team clinical education, enhancing team culture through open communication, utilizing and providing feedback on the electronic health record system, and participating in quality improvement projects.
