Responsibilities

Conduct In-Home Health Evaluations (approximately 45 minutes)
Engage with people one-on-one to answer health questions
Review medication and medical history
Perform a physical evaluation
Conduct diagnostic tests (if ordered by the person’s health plan, e.g., diabetic eye exams, spirometry)
Utilize an iPad and intuitive clinical workflow for evaluations
Provide education to people regarding their health
Conduct virtual visits as needed
Provide Signify Health services including Diagnostic & Preventive Services (e.g., spirometry, DEE, bone density screening)
