Requirements

Candidates must possess an active, unrestricted RN compact state licensure and have at least 3 years of clinical experience, particularly in Disease Management, Emergency Room, Critical Care, Medical/Surgical Care, or Oncology. Proficiency in Microsoft Office Suite, including Outlook and Word, is required, along with proficient keyboarding skills and experience with computers and evolving technology. Telephone experience in nursing, especially telephone triage, emergency room, or urgent care, is preferred. An Associate Degree is required, with a Bachelor's Degree or equivalent 3-5 years of clinical experience being preferred. Candidates must be self-motivated, willing to work every other weekend and 4 holidays per year, and able to complete a 6-week orientation. Managed care and previous coaching experience are preferred.

Responsibilities

The Health Coach Consultant will utilize a collaborative process to assess, plan, implement, and evaluate patient care, engaging and educating members to achieve optimal health. Responsibilities include facilitating effective healthcare decision-making, improving the quality of care and member satisfaction, and promoting the appropriate use of healthcare resources. The consultant will provide up-to-date health information to help members understand their health problems and options, assist in clarifying available choices, and help members actively participate with their providers. They will also promote healthy lifestyles, identify opportunities to lower personal health risks, prevent illness, and make referrals to available health and wellness programs as appropriate. The role involves applying broad clinical experience to complex issues, ensuring a positive member experience, and expanding knowledge while utilizing excellent communication skills. Candidates are expected to be leaders in people and process improvement and to act with integrity in all interactions.
