The Field Service Coordinator will assess and evaluate member needs and requirements to guide them toward optimal wellness, connecting them with appropriate resources. Responsibilities include administering assessments, contacting members telephonically and in-person to establish goals and develop care plans, and identifying LTSS providers. The role involves supporting members in navigating services, maintaining Medicaid eligibility, and collaborating with healthcare professionals to ensure holistic service delivery and positive member outcomes.
