Requirements

  • M.D., D.O., A.P.N., or P.A. graduate
  • Board certification in Internal Medicine or Family Medicine (required for M.D. or D.O.); Board certification in Family, Adult, or Gerontology (for A.P.N.)
  • Experience in clinical leadership roles, leading and coaching

Responsibilities

  • Work in tight collaboration with the Practice Manager and Outreach Manager/Director to achieve organizational objectives (Best Care Anywhere, Unmatched Patient Experience, Exceptional Growth, Great Place to Work, Leader in DE&I)
  • Drive implementation of OSH strategic initiatives
  • Follow direction from regional leadership to implement regional priorities and objectives; cascade organizational information to care team and escalate concerns/ideas
  • Help coordinate and lead onboarding of new providers
  • Model excellent medical care, care model implementation, and success in metrics
  • Support consistent OSH care model implementation (daily huddles, patient care planning/management, admission prevention, clinical documentation)
  • Ensure medical care is appropriate, evidence-based, keeps patients healthy/out of hospital
  • Understand, teach, and implement Oak Street's clinical guidelines at provider and center level
  • Support care teams/center in providing highest quality primary/secondary preventative care
  • Educate and support providers in value-based full risk model of care
  • Assess/address root causes when center metric goals not reached
  • Create/maintain culture of coaching up and growth for providers/care teams
  • Listen to feedback from care team and address concerns with constructive problem-solving
  • Conduct performance reviews, discuss provider goals, develop achievement plans
  • Acknowledge/celebrate successes, communicate improvement opportunities
  • Achieve triad best practices
  • Support positive "Oaky" culture of joy in practice
  • Other duties as assigned
  • Practice primary care in clinics (80% clinical time, 20% administrative time); serve as mentor/coach/advisor to providers; participate in organization-wide strategic initiatives
