Requirements

Candidates must have a high-school level education or equivalency and at least 2 years of experience in an outpatient, ambulatory clinic or virtual setting. A Medical Assistant Certification is preferred, and Spanish fluency is also preferred.

Responsibilities

The Care Team Assistant will provide clerical support and ensure the delivery of quality, compassionate, evidence-based care in a Value-Based Care environment. This includes accepting and returning phone calls, performing protocol-based triage, documenting calls in EHRS, placing calls and creating letters for lab results, and reviewing daily schedules for pre-visit preparations. They will also call patients at risk for no-shows, onboard new patients, initiate completion of forms for provider signature, monitor and respond to Teams chats, assist the care team with reports and identifying vulnerable populations, and collaborate with colleagues. Other duties as assigned will also be performed.
