Requirements

Candidates must possess a High School Diploma or GED, with experience resolving escalated customer service issues. Proficiency in Google Suite products, including Docs, Gmail, Calendar, Slides, and Sheets with basic analytical formulas, is required. Prior experience with phone/fax systems and EMR systems, preferably Athena and Epic, is necessary. Preferred qualifications include an Associate's Degree or higher, a typing speed of 40+ WPM, and experience in a healthcare clinical or administrative role.

Responsibilities

The Care Coordinator will answer inbound patient calls, transfer escalated issues, and provide excellent customer service by actively listening and resolving questions. Responsibilities include assisting patients with scheduling provider visits, collecting necessary documentation from patients and providers, and supporting care teams with non-clinical tasks. The role also involves capturing patient referrals via fax, accurately documenting patient interactions, and ensuring timely follow-up on assigned tasks.
