Responsibilities

  • Assist the RN in patient care by completing assigned patient care tasks, in accordance with the NYS LPN scope of practice
  • Collects patient healthcare data systematically and, on an ongoing basis, using appropriate assessment techniques and instruments
  • Administer medications and vaccinations (including injections)
  • Provide telephonic support for patients, scheduling problem visits or nurse visits when appropriate, offering symptom management under the direction of the provider, supporting refill requests and answering visit follow-up questions
  • Monitor and coordinate delivery of the care plan
  • Form relationships with patients and their caregivers to support prevention focused care and ED/ hospital diversion where appropriate
  • Conduct medication and testing authorizations
  • Deliver an exceptional patient experience through service, responsiveness and care
  • Other duties, as assigned
