- Current state license to practice as an LPN
- CPR Certification required
- EMR experience

- 2 or more years of healthcare experience working in a primary care office or other outpatient setting

Responsibilities
- Assist the RN in patient care by completing assigned patient care tasks, in accordance with the NYS LPN scope of practice
- Collects patient healthcare data systematically and, on an ongoing basis, using appropriate assessment techniques and instruments
- Administer medications and vaccinations (including injections)
- Provide telephonic support for patients, scheduling problem visits or nurse visits when appropriate, offering symptom management under the direction of the provider, supporting refill requests and answering visit follow-up questions
- Monitor and coordinate delivery of the care plan
- Form relationships with patients and their caregivers to support prevention focused care and ED/ hospital diversion where appropriate
- Conduct medication and testing authorizations
- Deliver an exceptional patient experience through service, responsiveness and care
- Other duties, as assigned
