Responsibilities

  • Provide In-Home Health Evaluations, including medication and medical history review, physical evaluation, and additional diagnostic tests if ordered (e.g., diabetic eye exams, spirometry)
  • Conduct visits lasting about 45 minutes using an iPad and intuitive clinical workflow
  • Bring heart into every visit and build trusted relationships to improve health outcomes
  • Be flexible to travel locally and within licensed states as needed
  • Conduct virtual visits as needed
  • Provide all Signify Health services as indicated, including Diagnostic & Preventive Services products (e.g., spirometry, DEE, bone density screening)
