Requirements

  • North Carolina residency is required
  • Willingness to travel within assigned DSS Region
  • Ability to work field-based 4-5 days per week interacting in person with patients, members, or providers
  • Ability to work virtually from home when not in the field
  • Within reasonable commuting distance from posting location(s) in specified DSS Regions (unless accommodation granted)
  • Scope of licensure for case management (telephonically and/or by home visits)

Responsibilities

  • Coordinate operations and workflows related to Intellectual, Developmental & Disability (IDD) case management activities in support of CFSP Foster Care
  • Serve as coach for field care managers, providing consulting on complex medical or IDD cases
  • Perform case management telephonically and/or by home visits within the scope of licensure
  • Manage overall healthcare costs for the designated population via integrated (physical health/behavioral health) case management and whole person health
  • Manage the most complex cases and provide support to Special Programs case managers
  • Engage collaboratively with key stakeholders, including families, foster parents, and service teams to craft and sustain individualized treatment plans
  • Conduct assessments to identify individual needs
  • Develop comprehensive care plans to address objectives and goals as identified during assessment
