Requirements

Candidates should possess a Bachelor’s Degree or equivalent work experience and have social work or case management experience in the disability or social services field. Client-facing experience managing multiple relationships is essential, along with excellent written and verbal communication skills. A reliable computer, high-speed internet connection, and a quiet work environment are required. Fluency in Spanish is strongly preferred, as is experience in a performance-based culture with metrics attainment goals. Experience serving in the developmental disability field, familiarity with Microsoft Office and Google Suite, and knowledge of person-centered planning in the California developmental disability system are also preferred, along with at least 2 years of case management experience.

Responsibilities

The Care Manager will engage in person-centered planning to capture clients' unique strengths and goals, translating personalized plans into concrete support needs while advocating for those needs to local budget authorities. They will manage projects, guiding clients and partner organizations throughout the process. The role involves leveraging proprietary resources to assist in implementing person-centered plans and contributing insights from client interactions into key company strategies and initiatives each quarter.
