Requirements

  • Bachelor's degree in health and human services field
  • 2 or more years of related experience
  • Knowledge of Microsoft Office Word and Excel
  • Exceptional verbal/written communication and interpersonal skills
  • Ability to use a variety of electronic information applications/software programs
  • Valid state driver's license and proof of personal vehicle liability insurance with at least 100,000/300,000/100,000 limits
  • Reside in Lawrence, Orange, Crawford, and Washington County Indiana

Responsibilities

  • Assesses and evaluates member's needs and requirements to achieve and/or maintain optimal wellness state
  • Connects members with quality services to promote their ultimate well-being and drive health outcomes
  • Administers initial and ongoing long-term services and support (LTSS) related assessments
  • Contacts members to establish goals and priorities, evaluate resources, develop plan of care and identify LTSS providers and community partnerships
  • Develops and continuously modifies Service Plan and involves applicable members of the care team in care planning
  • Supports members through navigation of their LTSS and related environmental and social needs
  • Uses available information pertaining to member to prevent the need for administration of duplicative assessments
  • Supports members and/or caregivers in accessing long term services and support, social, housing, educational and other services
  • Assisting members in maintaining Medicaid eligibility
  • Collaborates with Medical Director/Geriatrician/Care Coordinator to ensure cohesive, holistic service delivery and support positive member outcomes
