Candidates must possess a Bachelor's degree in a health and human services field and have at least 2 years of related experience. Proficiency in Microsoft Office Word and Excel, exceptional verbal and written communication skills, and the ability to use various electronic information applications are required. A valid state driver's license, proof of personal vehicle liability insurance with specific limits, and a passion for improving consumer experiences are also necessary. Prior experience with Medicare & Medicaid recipients, electronic case note documentation, health promotion, coaching, wellness, and knowledge of community health and social service agencies are preferred.
The Field Service Coordinator assesses and evaluates member needs to guide them towards optimal wellness, connecting them with appropriate resources. Responsibilities include administering LTSS assessments, contacting members telephonically and in-person to establish goals and develop care plans, and identifying LTSS providers and community partnerships. The role involves continuous modification of Service Plans, supporting members in navigating LTSS and related needs, assisting with Medicaid eligibility, and collaborating with medical teams for cohesive service delivery.
