Diabetes NP Manager

Position Overview

Devoted Health is seeking an ambitious Nurse Practitioner (NP) to take on a new leadership role within the Devoted Medical Diabetes Clinic. This position offers an opportunity to contribute to the clinical excellence of the team by providing clinical and operational oversight of NPs, while also delivering direct patient care. The Diabetes NP Manager will be part of the specialty care management team, working collaboratively to strengthen the program, empower staff, and improve patient care. This role reports to the Medical Director for Specialty Care.

The Diabetes Clinic is a virtual specialty care program designed as a "micro center of excellence" to deliver specialized care to patients with diabetes and its associated comorbidities. The NP will provide whole-person care and lead the clinical management of complex patients.

Responsibilities

70% Patient Care (24 clinical hours per week):

• Provide excellent virtual care to patients with diabetes as part of an interdisciplinary team, including Certified Diabetes Educators (CDEs), social workers, and clinical pharmacists.
• Optimize glycemic control by initiating and titrating diabetes medications according to clinical guidelines.
• Conduct thorough assessments to address micro and macrovascular complications in patients with diabetes.
• Order and interpret diagnostic tests, labs, and imaging data to develop comprehensive treatment plans.
• Coordinate care by being responsive to management questions from CDEs.

30% Diabetes NP Manager Duties:

• Provide oversight and accountability for the clinical performance of the diabetes NP team, including quality chart reviews and coaching.
• Lead and maintain a strong team culture, embodying collaboration and engagement.
• Act as a liaison to the clinic manager and medical director on patient care, interdisciplinary coordination, and performance management.
• Lead hiring, interviewing, onboarding, and ongoing training for the diabetes NP team.
• Participate in essential clinical leadership meetings (interdisciplinary team meetings, clinical oversight meetings, clinic leadership team meetings, etc.).
• Identify areas for improvement and implement strategies to achieve targeted metrics.
• Oversee licensure of direct reports to ensure adequate clinical coverage.
• Manage, empower, inspire, and align the growing team of NPs, including conducting regular one-on-one meetings and setting personalized goals.
