Requirements

Trained and credentialed for all health plan clients and lines of business in primary coverage state(s)
Provide at least 12 hours of availability weekly, consistently providing 50+ capacity units per month
Execute a strong visit completion rate, pacing 35+ completed visits per month
Option to provide availability Monday - Sunday, with majority of visits between 8 a.m. and 7 p.m
Maintain active state licenses in coverage area
Maintain board certification
Maintain multi-state licenses (preferred)
Experience with Signify Health In-Home Health Evaluations, in-home visits, or similar format (preferred)
Flexible to travel locally and within licensed states as needed (up to 25%)
Ability to sit, stand, bend, reach, push, pull, and lift up to 20 pounds of supplies (e.g., iPad, medical supplies, scale)
Accommodate field observations and/or “ridealong” experiences as requested

Responsibilities

Conduct pre-scheduled in-home visits with health plan members, traveling primarily locally within a designated metro area
Conduct Video Visits as needed, maintaining training, affiliation, and compliance (regular training required)
Maintain training and complete all diagnostic and preventive service (DPS) products as directed (regular training required)
Maintain all required supplies and associated diagnostic equipment related to DPS offerings
Be accountable to Signify Health operational, clinical, and member experience key performance indicators and metrics
During In-Home Health Evaluations (approx. 45-minute visits): review medication and medical history, perform physical evaluation, and conduct additional diagnostic tests if ordered (e.g., diabetic eye exams, spirometry)
Use pre-populated, patient-specific forms for each evaluation (no prescribing medicine, ordering lab tests, or altering treatment regimens)
