Requirements

  • Trained and credentialed for all health plan clients and lines of business in primary coverage state(s)
  • Provide at least 12 hours of availability weekly, consistently providing 50+ capacity units per month
  • Execute a strong visit completion rate, pacing 35+ completed visits per month
  • Option to provide availability Monday - Sunday, with majority of visits between 8 a.m. and 7 p.m
  • Maintain active state licenses in coverage area
  • Maintain board certification
  • Maintain multi-state licenses (preferred)
  • Experience with Signify Health In-Home Health Evaluations, in-home visits, or similar format (preferred)
  • Flexible to travel locally and within licensed states as needed (up to 25%)
  • Ability to sit, stand, bend, reach, push, pull, and lift up to 20 pounds of supplies (e.g., iPad, medical supplies, scale)
  • Accommodate field observations and/or “ridealong” experiences as requested

Responsibilities

  • Conduct pre-scheduled in-home visits with health plan members, traveling primarily locally within a designated metro area
  • Conduct Video Visits as needed, maintaining training, affiliation, and compliance (regular training required)
  • Maintain training and complete all diagnostic and preventive service (DPS) products as directed (regular training required)
  • Maintain all required supplies and associated diagnostic equipment related to DPS offerings
  • Be accountable to Signify Health operational, clinical, and member experience key performance indicators and metrics
  • During In-Home Health Evaluations (approx. 45-minute visits): review medication and medical history, perform physical evaluation, and conduct additional diagnostic tests if ordered (e.g., diabetic eye exams, spirometry)
  • Use pre-populated, patient-specific forms for each evaluation (no prescribing medicine, ordering lab tests, or altering treatment regimens)
