This is not a remote role; it requires conducting pre-scheduled in-home visits with health plan members, traveling primarily locally within a designated metro area.
Candidates must maintain active state licenses in the coverage area, board certification, and multi-state licenses (preferred), along with experience in in-home health evaluations preferred. Physical ability to sit, stand, bend, reach, push, pull, and lift up to 20 pounds is required, and flexibility for local travel up to 25% is needed.
The role requires providing at least 12 hours of availability weekly and consistently providing 50+ capacity units per month, pacing 35+ completed visits per month.
Visits are approximately 45-minute pre-scheduled in-home evaluations including a medication and medical history review, a physical evaluation, and if ordered, additional diagnostic tests.
