Your Role

We are seeking a dedicated individual with experience in mental health crisis response. In this role, you will provide direct outreach to individuals utilizing innovative technology. You will be instrumental in delivering response services and contributing to the ongoing monitoring and enhancement of these services.

Responsibilities

  • Conduct outreaches via phone, email, and text messaging to patients requiring mental health support.
  • Employ empathy and active listening skills to build rapport, engage, and provide brief support to at-risk individuals.
  • Utilize the Columbia Suicide screening tool and safety planning techniques to assess risk levels and provide appropriate support.
  • Assist in maintaining a database of available referrals and resources to meet patients' mental health and community needs.
  • Contribute to the development of NeuroFlow digital health content and assign relevant content to patients.
  • Maintain timely and accurate documentation of all patient communications and services rendered.
  • Coordinate with NeuroFlow staff, customers, and emergency services as necessary to ensure patient safety.
  • Collaborate with customers and teletherapy partners regarding patient follow-up and referral status.
  • Competently use the NeuroFlow platform for patient communication, progress tracking, and documentation.
  • Assist in tracking data and outcomes to improve the quality of response services.
  • Support program accreditation achievement and maintenance of program standards.
  • Participate in monthly supervision sessions with the Clinical Program Manager.

About You

  • You possess at least a Bachelor's degree in psychology, nursing, social work, or a related field.
