Requirements
- Be trained and credentialed for all health plan clients and lines of business in primary coverage state(s)
- Be accountable to Signify Health operational, clinical, and member experience key performance indicators and metrics
- Conduct Video Visits as needed (maintain training, affiliation, and compliance with Video Visit requirements, regular training required)
- Maintain training and complete all diagnostic and preventive service (DPS) products as directed (regular training required)
- Maintain all required supplies and associated diagnostic equipment related to DPS offerings
- Maintain active state licenses in coverage area
- Maintain board certification
- Maintain multi-state licenses (preferred)
- Experience with Signify Health In-Home Health Evaluations, in-home visits, or a similar format (preferred)
- Ability to sit, stand, bend, reach, push, pull and lift up to 20 pounds of supplies (including, but not limited to, iPad, medical supplies and scale), required
- Be flexible to travel locally and within licensed states, as needed (up to 25%)
Responsibilities
- Conduct pre-scheduled in-home visits with health plan members, traveling primarily locally within a designated metro area
- Provide at least 12 hours of availability weekly, consistently providing 50+ capacity units per month
- Execute a strong visit completion rate, pacing 35+ completed visits per month
- Have the option to provide availability Monday - Sunday, with the majority of visits scheduled between 8 a.m. and 7 p.m
- Conduct Video Visits as needed
- Maintain training and complete all diagnostic and preventive service (DPS) products as directed
- Meet people where they are and re-engage them with their health care during an approximately 45-minute pre-scheduled visit
- The visit includes a medication and medical history review, a physical evaluation, and, if ordered by the person’s health plan, additional diagnostic tests (i.e., diabetic eye exams)
