Clinical Care Navigator at Lyra Health

As a Clinical Care Navigator, the primary responsibility is to manage client crises by utilizing evidence-based interventions, conducting brief risk assessments, developing safety plans, and de-escalating situations with the goal of connecting clients to appropriate treatment. This role involves identifying and overcoming barriers to accessing clinical care, independently managing a caseload of complex clients, and providing in-the-moment phone support.
