Requirements

Candidates must hold a Master’s degree in clinical social work or an equivalent degree, and possess an independent clinical license such as LCSW, LMFT, LPC, or LPCC. Alternatively, candidates with a Bachelor’s degree in nursing (RN) and two years of psychiatric intake and assessment experience are eligible. Bilingual Spanish-speaking candidates are strongly preferred, though not required.

Responsibilities

As a Clinical Care Navigator, the primary responsibility is to manage client crises by utilizing evidence-based interventions, conducting brief risk assessments, developing safety plans, and de-escalating situations with the goal of connecting clients to appropriate treatment. This role involves identifying and overcoming barriers to accessing clinical care, independently managing a caseload of complex clients, and providing in-the-moment phone support. The position requires skilled communication, time management, and crisis management abilities, as well as a commitment to connecting individuals with high-quality mental health care.
