The Conditioning Health Coach will prioritize and manage individual members through their health journey, delivering remote coaching via apps and video conferencing. They will engage and activate members to set goals, manage their care, and stay motivated, while providing exceptional service and support. Responsibilities include daily monitoring of assigned members for program compliance, reviewing health data and reports for accuracy and adherence, and providing feedback on daily metrics such as weight, glucose monitoring, blood pressure, sleep, breathing, movement, diet, and supplement adherence.
