Candidates must be fluent in Spanish and English, demonstrating strong communication skills. Prior experience in case management, particularly with individuals experiencing homelessness, Serious Mental Illness, or Substance Use Disorder is preferred. A Bachelor's degree in social work, psychology, or a related field is desirable.
The Lead Care Manager will be responsible for directly outreaching and engaging with individuals experiencing homelessness, Serious Mental Illness, or Substance Use Disorder, building trust and establishing relationships. They will play a critical role in our whole-person, interdisciplinary care model, coordinating care across various services and collaborating with a team of PairMates. The role involves on-site visits in the community 1-2 times per month, and contributing to the development and implementation of care strategies.
Connects vulnerable patients with local care teams
Pair Team connects vulnerable patients with local care teams that provide physical, mental, and social services. They achieve this by partnering with large healthcare providers, hospitals, health systems, municipalities, shelters, and social support services. The company focuses on whole-person care, offering their services for free to patients by coordinating resources across their partner network. This model is funded through the Enhanced Care Management benefit of Medi-Cal, ensuring no cost to patients. Additionally, Pair Team compensates community partners for their involvement in patient care, creating a revenue stream for these organizations. They also provide extra support like virtual urgent care, free grocery delivery, and housing navigation, which enhances patient care and eases the workload for healthcare staff. Pair Team stands out by specifically targeting vulnerable populations and collaborating closely with community organizations to deliver comprehensive care.
