Responsibilities

  • Act as primary call point for spoke hospitals aligned with PPD hubs and regional single sites, fostering strong relationships and consistent communication
  • Implement spoke account plans, executing regional account strategies focused on patient escalation, movement within health systems, and establishing protocols for heart team transport and patient flow
  • Develop regional pathways for patients with CAD and/or heart failure for advanced therapy evaluation, including Impella protected procedures
  • Align spoke heart team and transport protocols with hub protocols and internal standards; share best practices for patient care pathways identifying high-risk patients and flagging need for ischemic evaluation
  • Maintain clear communication channels for visibility into hub protocols and regional activities, ensuring stakeholder alignment
  • Develop and execute regional and single-site account plans to support program growth and sustainability
  • Collaborate with internal teams and hospital stakeholders for alignment on goals, protocols, and program activities
  • Provide education on disease states for patient identification in outpatient and inpatient settings, including heart failure clinics, emergency medicine departments, and cardiovascular practices
