Requirements

  • Functions under direct supervision of a licensed nurse
  • Compliance with organization policies, procedures, training, and state/federal regulations
  • Knowledge of dialysis therapy, including Online Clearance Monitoring (OLC), Adequacy Monitoring Program (AMP), Urea Kinetic Modeling (UKM), disease process/access, vascular access cannulation, heparin administration (as delegated or allowed by state law)
  • Ability to setup and operate hemodialysis machines
  • Ability to perform vital signs, weights, vascular access evaluation, dialysis initiation/discontinuation, intradialytic monitoring/interventions, hemostasis, dressings, UF goal calculation, machine programming, safety alarms (e.g., Narrow Venous Limits 160), laboratory testing/specimen preparation
  • Ability to maintain safe/clean environment, conduct machine safety checks, clean blood spills
  • Accurate and timely documentation skills (e.g., Hemodialysis Treatment Sheet, progress notes)
  • Observation and reporting skills for patient conditions/changes to RN/supervisor
  • Participation in Quality Enhancement Program (QEP), CQI activities, process improvements, patient satisfaction

Responsibilities

  • Assist in patient education on adequacy measures (OLC, AMP, UKM) and disease/access
  • Welcome patients, inquire about wellbeing, evaluate vascular access patency, cannulate access, administer heparin (as allowed)
  • Obtain pre/post treatment vital signs, weights, vascular access evaluations
  • Initiate dialysis per orders (QB, QD), monitor response, evaluate intradialytic problems, intervene per physician/RN direction
  • Monitor patients during reinfusion, discontinue treatment per procedures, evaluate pre-discharge (standing/sitting BP)
  • Perform/record pre/post dialysis evaluations, weights, vital signs; calculate/enter UF goal, treatment time, UF modeling
  • Ensure safety alarms enabled, record vital signs/treatment parameters accurately
  • Document all treatment info, complete Hemodialysis Treatment Sheet/progress notes
  • Report significant patient changes/unusual findings to RN/supervisor
  • Conduct machine safety checks, intervene/document unusual patient status
  • Monitor vascular access (arterial/venous pressures), intervene/report as needed
  • Obtain lab tests, prepare specimens, clean blood spills per policy
  • Prepare/organize supplies/equipment efficiently to prevent waste
  • Monitor self-care patients under RN supervision
  • Assist in training direct patient care staff on programs
  • Support QEP/CQI activities and process improvements for quality goals
