Requirements

  • Entry level designation into the Clinical Advancement Program (CAP)
  • Ability to perform all essential functions under the direction of the Supervisor and with guidance from the Clinical Educator, Preceptor, or in collaboration with other Registered Nurses
  • Capable of ongoing systematic collection and analysis of patient data pre-during-post hemodialysis treatment, documentation in patient medical record, and adjustments to treatment plan
  • Ability to assess, collaborate, and document patient/family’s basic learning needs for initial and ongoing education
  • Skills to direct and provide, in collaboration with direct and ancillary patient care staff, all aspects of daily safe and effective hemodialysis therapy delivery
  • Competence in administering medications as prescribed or per approved algorithm(s), with documentation of justification and effectiveness
  • Ability to initiate or assist with emergency response measures
  • Capability to serve as a resource for the health care team and participate in staff training and orientation
  • Knowledge of correct laboratory collection, processing, and shipping procedures, including rescheduling missed or insufficient collections
  • Ability to identify expected outcomes, document and update nursing assessment and plan of care through collaboration with the Interdisciplinary Team
  • Required to complete CAP requirements to advance
  • Physical ability for direct patient care involving heavy lifting, moving patients, assisting with ambulation, frequent prolonged standing, bending, moving machines/equipment up to 200 lbs. with assistance, lifting chemical/water solutions up to 30 lbs. as high as 5 feet
  • Tolerance for work environment with air temperature control, moderate noise, exposure to infectious/contagious diseases, desk/computer work, patient/staff/physician interaction, and potential travel
  • Compliance with Fresenius Kidney Care policy, procedures, standards of nursing practice, state and federal regulations

Responsibilities

  • Provision and coordination of clinically competent care including assessment, planning, intervention, and evaluation for assigned patients
  • Delegation of appropriate tasks to direct patient care staff (RNs, LVN/LPNs, Patient Care Technicians)
  • Participation in decision-making, teaching, leadership functions, and quality improvement activities as part of the End Stage Kidney Disease (ESKD) health care team to enhance patient care outcomes and facility operations
  • Perform ongoing systematic collection and analysis of patient data pre-during-post hemodialysis treatment for assigned patients, document in medical record, make adjustments/modifications to treatment plan, and notify Team Leader, Charge Nurse, Supervisor, or Physician as needed
  • Assess, collaborate, and document patient/family’s basic learning needs to provide initial and ongoing education
  • Direct and provide, in collaboration with direct and ancillary patient care staff, all aspects of daily safe and effective hemodialysis therapy delivery to assigned patients
  • Administer medications as prescribed or per approved algorithm(s) and document appropriate medical justification and effectiveness
  • Initiate or assist with emergency response measures
  • Serve as a resource for health care team, participate in staff training and orientation of new staff as assigned
  • Ensure correct laboratory collection, processing, and shipping procedures; reschedule missed or insufficient collections
  • Identify expected outcomes, document and update nursing assessment and plan of care for assigned patients through collaboration with the Interdisciplinary Team
  • Ensure patient awareness related to transplant and treatment modality options
  • Complete CAP requirements to advance
  • Perform all other duties as assigned by Supervisor
