The LTSS Service Coordinator will conduct face-to-face program assessments for individuals, utilizing motivational interviewing techniques to identify needs and preferences, facilitate the Person-Centered Planning process, develop comprehensive PCSPs and backup plans, and coordinate medical, behavioral health, and LTSS services. They will also manage non-clinical needs, document member goals, identify potential clinical healthcare needs, and serve as a single point of contact for members, collaborating with care teams and submitting utilization reports.
