Requirements

Board-certified nurse practitioner (FNP, AGNP certification preferred)
Maintain active state licenses in coverage areas (multi-state licenses preferred)
Maintain board certification
Trained and credentialed for all health plan clients and lines of business in primary coverage state(s)
Provide 40 hours of availability weekly, consistently providing 160+ capacity units per month
Execute a strong visit completion rate, consistently pacing 110+ completed visits per month
Experience with Signify Health In-Home Health Evaluations, in-home visits, or similar format (preferred)
Be flexible to travel locally and within licensed states (may require hotel stay, car rental, and/or flight up to 20% annually)
Ability to sit, stand, bend, reach, push, pull and lift up to 20 pounds of supplies
Accommodate field observations and/or “ridealong” experiences, as requested
Maintain training and complete all diagnostic and preventive service (DPS) products as directed (regular training required)
Conduct Video Visits as needed (maintain training, affiliation, and compliance with Video Visit requirements, regular training required)
Maintain all required supplies and associated diagnostic equipment related to DPS offerings
Consistently engage with regular manager, team and clinical meetings
Be accountable to Signify Health operational, clinical, and member experience key performance indicators and metrics

Responsibilities

Conduct pre-scheduled in-home visits with health plan members (Medicare Advantage, Medicaid, and certain commercial health plan members), traveling primarily locally within a designated metro area (e.g., Los Angeles, Orange, and Ventura counties; majority of visits scheduled weekdays between 8 a.m. and 7 p.m.)
Perform medication and medical history review, physical evaluation, and (if ordered) additional diagnostic tests (e.g., diabetic eye exams, spirometry) during approximately 45-minute visits
Use pre-populated, patient-specific forms for each evaluation (no prescribing medicine, ordering lab tests, or altering treatment regimens required)
