Multi-state licenses (preferred)
Able to cover the following primary locations: Rochester, MN and surrounding area
Able to comfortably and safely perform duties in diverse home environments that may contain household pets, secondhand smoke, and other potential airborne allergens or irritants
Flexible to travel locally and within licensed states, as needed

Responsibilities

Provide In-Home Health Evaluations, including medication and medical history review, physical evaluation, and additional diagnostic tests if ordered (e.g., diabetic eye exams, spirometry)
Conduct visits lasting about 45 minutes, connecting one-on-one, answering health questions, using an iPad and intuitive clinical workflow
Bring heart into every visit, building trusted relationships to improve health outcomes
Work with Signify Health as a clinician colleague, fulfilling the requirements of the specified role
Conduct virtual visits as needed
Provide all Signify Health services as indicated, including Diagnostic & Preventive Services products (e.g., spirometry, DEE, bone density screening)
