Active, unrestricted license(s) in coverage area(s) (MDs and DOs)
Multi-state licenses (preferred)
Able to cover the following primary locations: Monterey, San Benito, Santa Clara, and Santa Cruz counties

Responsibilities

Provide In-Home Health Evaluations, including medication and medical history review, physical evaluation, and additional diagnostic tests if ordered (e.g., diabetic eye exams, spirometry)
Conduct visits lasting about 45 minutes using an iPad and intuitive clinical workflow
Bring heart into every visit, building trusted relationships to improve health outcomes
Be flexible to travel locally and within licensed states as needed
Conduct virtual visits as needed
Provide all Signify Health services as indicated, including Diagnostic & Preventive Services products (e.g., spirometry, DEE, bone density screening)
