Multi-state licenses (preferred)
Able to cover the following primary locations: Bronx and surrounding area
Able to comfortably and safely perform duties in diverse home environments that may contain household pets, secondhand smoke, and other potential airborne allergens or irritants
Flexible to travel locally and within licensed states, as needed

Responsibilities

Provide In-Home Health Evaluations, including medication and medical history review, physical evaluation, and additional diagnostic tests if ordered (e.g., diabetic eye exams, spirometry)
Conduct visits lasting about 45 minutes, connecting one-on-one and answering health questions
Use an iPad and intuitive clinical workflow for each evaluation (no prescribing medicine, ordering lab tests, or altering treatment regimens required)
Bring heart into every visit and build trusted relationships to improve health outcomes
Conduct virtual visits as needed
Provide all Signify Health services as indicated, including Diagnostic & Preventive Services products (e.g., spirometry, DEE, bone density screening)
