Active, unrestricted license(s) as Nurse Practitioner or Physician Assistant in coverage area(s), specifically able to cover Manhattan, NY
Board certification (required)
Multi-state licenses (preferred)
Able to work part-time (anticipated 29.5 weekly hours)
Flexible to travel locally and within licensed states, as needed
Able to conduct virtual visits as needed

Responsibilities

Conduct in-home health evaluations lasting about 45 minutes, including medication and medical history review, physical evaluation, and additional diagnostic tests if ordered (e.g., diabetic eye exams, spirometry)
Use an iPad and intuitive clinical workflow for each evaluation
Bring empathy and build trusted relationships during visits to improve health outcomes
Provide all Signify Health services as indicated, including Diagnostic & Preventive Services products (e.g., spirometry, DEE, bone density screening)
Conduct virtual visits as needed
Engage one-on-one, answer health questions, and connect people to appropriate care without prescribing medicine, ordering lab tests, or altering treatment regimens
