This is a full-time, 5-day, 8-hour position from 8:30 AM to 5:00 PM in CST, PST, or MST, including one weekend shift per month. Candidates must consistently work within Pacific Standard Time (PST), Mountain Standard Time (MST), or Central Standard Time (CST) hours.
Responsibilities include performing initial TOC visits with comprehensive post-discharge assessments, medication reconciliation, orders, and education; conducting acute symptom visits for diagnosis and management; participating in interdisciplinary rounds; collaborating with RNs, LPNs, and coordinators; assisting with team education; and enhancing team culture.
The clinical team is highly interdisciplinary, composed of coordinators, guides, nurse case managers, nurse practitioners, and a medical director. The role involves significant collaboration with nurse case managers, clinical guides, clinical coordinators, RNs, and LPNs.
This is a virtual care position providing post-discharge care remotely within specified time zones.
Strong candidates should have experience conducting comprehensive post-discharge assessments, developing care plans, providing disease-specific education, diagnosing and managing conditions like heart failure, COPD, diabetes, and others, and collaborating in interdisciplinary teams.
