Responsibilities

Conduct pre-scheduled in-home visits with health plan members (45-minute visits including medication and medical history review, physical evaluation, and additional diagnostic tests if ordered, e.g., diabetic eye exams, spirometry)
Use an iPad and intuitive clinical workflow for each evaluation
Provide all Signify Health services including Diagnostic & Preventive Services products (e.g., spirometry, DEE, bone density screening)
Conduct virtual visits as needed
Bring heart into every visit, building trusted relationships to improve health outcomes and connect people to the right care
