Board certification (required)
Able to cover the following primary locations: Syracuse, NY - Oneida/Onondaga/Oswego Counties
Multi-state licenses (preferred)
Flexible to travel locally and within licensed states, as needed

Responsibilities

Conduct in-home health evaluations (approximately 45 minutes each), including medication and medical history review, physical evaluation, and additional diagnostic tests if ordered (e.g., diabetic eye exams, spirometry)
Use an iPad and intuitive clinical workflow for each evaluation
Bring heart into every visit, building trusted relationships to improve health outcomes
Conduct virtual visits as needed
Provide all Signify Health services as indicated, including Diagnostic & Preventive Services products (e.g., spirometry, DEE, bone density screening)
Work flexibly with a national network of clinicians, fulfilling the requirements of the specified role
